A home care assessment is a comprehensive evaluation conducted to determine a person’s care needs and create a personalized care plan. A Paragon representative will visit your residence to capture your care needs, daily tasks and activities, medication needs, and any other requests or preferences to help us find the ideal caregiver match for the assignment. This is a conversational and collaborative process that helps maximize the client’s experience with home care.
